Psychosocial Rehabilitation Provider
(PSR)
Ever Well Community Health provides various services to children, adolescents, and adults that include: Therapeutic Behavioral Services, Psychosocial Rehabilitation Services, Community Psychiatric Support and Treatment, Behavioral Health Counseling, Mental Health Assessment, Treatment Planning, Medication Management (18+), and Nursing Services.

Job Summary
You will provide recovery-oriented rehabilitation services on an individual basis in a community setting. PSRs serve all of our programs and may have a variety of clients, or may be specialized based on experience (Youth, Adults, Senior Citizens, DD, etc.)
Job Duties/Responsibilities

Provides rehabilitative services through behaviorally based supportive activities

Assist clients with activities of daily living critical to remaining successful at home, school, work, and the community

Assessment of problems in the client’s life domains posing barriers to recovery
-
Balance time between visiting clients in the community (home, school, community center, etc.) and home office hours.

Assisting and linking clients to community resources to meet basic needs
-
Advocating and helping the client navigate the system to support access to needed resources

Educating the client regarding rehabilitative activities of daily living in order to guide the client to reach goals, objectives and treatment outcomes

Collaboration with client’s family members for the direct benefit of the client

Collaboration with referral sources and community agencies assisting the client

Stays knowledgeable of standards pertaining to service delivery

Establish and build rapport with clients who are experiencing mental or emotional issues

Implement and review treatment plans

Ensure all documentation is completed in a timely manner (within 48 hours)

Work with the rest of the client’s team and communicate effectively with colleagues
-
Maintain weekly productivity when appropriate
-
Transport clients if needed
-
Attend all required staff meetings and trainings
-
Other duties as assigned
Qualifications Requirements

A high school diploma with a minimum of three years of relevant experience OR currently pursuing a Bachelor’s or Master’s Degree in social work, psychology, nursing, or in related human services field

Valid Ohio Drivers License and insurance

Must be able to build rapport with clients

Excellent written and verbal communication skills

Familiarity with community mental health resources

Able to participate in First Aid, CPR, Verbal de-escalation training

Experience with, or ability to learn Google Suite
